If you searched this after noticing a supplement in your cabinet and a story online, here is the direction of the answer: ashwagandha has been linked to liver injury in a small number of case reports, the injury is usually reversible when the supplement is stopped, and it appears to be uncommon relative to how many people take the herb. That is not the same as saying it is safe for you specifically, and it is not the same as saying the risk is understood. What follows is a frame for telling the difference between a symptom to watch, a symptom to change your dose over, and a symptom for a clinic.

What does "ashwagandha liver damage" actually refer to?

The concern is a pattern called herbal-induced liver injury: liver inflammation triggered by a botanical product rather than a drug, alcohol, or a virus. Ashwagandha (Withania somnifera) is one of several botanicals that has been named in published case reports of this pattern. How serious are the reported cases, and how common are they?

The documented pattern in ashwagandha case reports is typically a cholestatic or mixed liver injury – jaundice, dark urine, itching, fatigue – appearing weeks after starting the supplement, and resolving after it is stopped. Most reported cases recover once the herb is withdrawn. Severe outcomes have been described but are rare in the published record.

On the exposure side, an analysis of US adults estimated how many people are taking botanicals with hepatotoxic potential, ashwagandha among them, and the figure is not trivial – millions of adults report use. That combination – wide use, few published injury reports – is the reason most people who take ashwagandha never experience a liver problem. It is also the reason the true rate cannot be read off the case reports alone: mild or unreported events do not reach the literature.

What did the controlled trial evidence show?

Trial evidence is thinner than the attention this topic gets. A randomised, placebo-controlled safety study in healthy volunteers assessed ashwagandha root extract and reported it was well tolerated over the study period, without the liver signal that the case reports would lead you to expect. This is genuinely reassuring for the population it studied – healthy adults, a defined extract, a limited duration – and genuinely limited for the same reasons. A short trial in healthy people is not designed to catch a rare, idiosyncratic reaction that may take weeks to appear and may depend on the individual, the specific product, or an interaction. Absence of a signal in a small trial is not proof of absence of risk.

A critical review of ashwagandha's potential adverse effects, drawing on both preclinical and clinical evidence, treats liver injury as a real but uncommon concern rather than a settled property of the herb. The honest reading is that controlled data and case data point in different directions because they are answering different questions: trials measure the average healthy user over weeks, case reports capture the rare individual who reacted.

A risk factor is not a cause

This is the section to slow down on, because most of what circulates online blurs it. Several things get named as reasons ashwagandha "damages the liver" – higher doses, concentrated extracts, taking it alongside medications, pre-existing liver conditions. Some of these are biologically plausible. None of them has been established as a cause of injury from the human data currently available.

  • Product quality and adulteration. A symposium report on botanical-induced toxicity highlights that what is on the label is not always what is in the capsule. Contamination and misidentification are documented problems for botanicals generally. This is a plausible contributor to some injury cases, not a proven mechanism for ashwagandha specifically.
  • Botanical–drug interactions. The same source flags interactions between botanicals and medications as a route to liver stress. Mechanistically reasonable; the human evidence tying a specific ashwagandha–drug pair to injury is limited.
  • Dose and extract concentration. Frequently blamed, and intuitive, but the case reports do not establish a threshold dose, and a clear dose–response relationship for injury has not been demonstrated.

Treat each of these as a hypothesis with a mechanism attached, not as a rule you can act on with precision. The one that is best supported – that product contents are not always what the label claims – is an argument for third-party tested products, not for any particular dose.

Which symptoms are worth acting on?

Most people will never need this list. It exists so that if something does change, you are not guessing.

  • Yellowing of the skin or the whites of the eyes.
  • Dark urine or pale stools.
  • Persistent itching without a rash.
  • Right-sided upper abdominal pain.
  • Unusual fatigue, loss of appetite, or nausea that does not settle.

If you develop any of these while taking ashwagandha, the sensible first step is to stop the supplement and have your liver function checked. This is not a load-and-recovery question you manage yourself; it is a reason to be assessed. Because reported cases usually resolve after withdrawal, stopping early matters.

Who should be more cautious than the average user?

Named narrowly, because "talk to your doctor" alone tells you nothing:

  • People with existing liver disease – the margin for an idiosyncratic reaction is smaller, and this is a decision to make with a clinician before starting, not after.
  • People taking medications metabolised by the liver – the interaction risk is plausible enough to raise with a prescriber, particularly if you take several drugs.
  • Anyone stacking multiple botanical supplements – if a problem arises, attribution becomes almost impossible, which is exactly the situation the injury-identification literature describes as hardest to manage.

For a healthy adult with no liver history, taking a single, quality-controlled product at a conventional dose, the picture the evidence supports is: uncommon risk, usually reversible, worth knowing the warning signs for.

What is still unknown

Openly: the true incidence of ashwagandha liver injury is not established, because the denominator – how many people take it, at what dose, for how long – is uncertain and the numerator relies on cases that reach the literature. Whether specific extracts, doses, or drug combinations raise the risk has not been demonstrated in humans. And the controlled safety data, while reassuring, come from short trials in healthy people that are not built to detect rare reactions. None of this makes the herb dangerous. It makes the honest position "uncommon, usually reversible, incompletely characterised" – and it makes knowing the warning signs the most useful thing you can take from this page.

The threshold to leave with

If you take ashwagandha and feel fine, the evidence does not demand that you stop. If you develop jaundice, dark urine, persistent itching, or unexplained fatigue, stop and get your liver checked rather than waiting it out. If you have liver disease or take liver-metabolised medication, make the decision to start with a clinician, not alone. One threshold, one trigger, and a clear sense that a symptom alone cannot tell you which category you are in.
